High-Intensity Personal Care

High-Intensity Daily Personal Activities

At Active Home Care Nurses, we are a registered provider delivering high-quality, high-intensity support for participants with complex care needs. These services are designed to ensure participants receive safe, respectful, and clinically informed care from qualified and trained staff.

What Are High-Intensity Supports?

High-Intensity Daily Personal Activities involve specialised care that goes beyond standard personal support due to the risk and complexity of the participant’s needs. These supports require trained workers to carry out or assist with:

  • Tracheostomy care and ventilation
  • Enteral (PEG) feeding
  • Complex bowel care
  • Urinary catheter management
  • Wound care
  • Severe dysphagia support
  • Subcutaneous injections (e.g. insulin)
  • Palliative care needs

These supports must be delivered in line with the Practice Standards and the High Intensity Support Skills Descriptors (HISSD).

Our Approach to High-Intensity Care

  • Trained and supervised by registered nurses or health professionals
  • Experienced in managing complex health conditions
  • Committed to promoting participant safety, dignity, and independence

Our team regularly updates care plans and conducts risk assessments in collaboration with participants, families, and clinical specialists.

How We Ensure Quality and Safety

  • Comprehensive individualised care planning
  • Ongoing training and supervision of support staff
  • Clear documentation aligned with High-Intensity requirements
  • Regular reviews and clinical oversight
  • 24/7 availability for high-risk participants (if required)

Who Is Eligible?

Participants may be eligible for high-intensity supports if they:

  • Require ongoing management of a complex health condition
  • Need care that poses a risk if not provided correctly
  • Have been assessed by their healthcare team or planner

Why Choose Us?

  • Registered for High-Intensity Daily Personal Activities (Registration Group 104)
  • Led by a team with clinical expertise and deep community experience
  • Trusted by families, support coordinators, and health professionals
  • Transparent, reliable, and participant-led care model
